Position Overview:
The Black Health Center of Excellence Case Manager position focuses on providing services for patients at the SFGH HIV clinic, Ward 86 and the UCSF 360 Clinic.  This position functions within a 3-person team and provides intensive case management and counseling services and group support to “high need” African Americans living with HIV/AIDS to facilitate access of individuals to HIV medical care and treatment, mental health services, substance abuse services and other culturally appropriate supportive services. The focus is on assisting the client in successfully engaging and maintaining with medical care & treatment, and a special focus on medication adherence.
 
 
Essential Duties and Responsibilities:
 

  1. Clinical & Case Management Services: Develop and maintain a clinically appropriate therapeutic relationship with the client, to help link the client to needed services, resources and opportunities. Conduct client eligibility screenings, including verifying severe-need status. Engage clients with medical care & treatment. Conduct psychosocial needs assessments every 6 months with clients and establish case management goals. Provide referrals regarding mental health, substance abuse and other support services and also provide one-on-one counseling and group support as appropriate. Provide support to clients for enhancing/maintaining medication adherence and conducting retention activities to ensure continued case management support.
  2. Consultation: Work within a multidisciplinary team, including San Francisco General Hospital & UCSF medical social workers, nurses and primary care physicians, via case conferences, verbal consultations, and progress notes in the electronic database.
  3. Outreach: Collaborate with other agencies, case managers and medical providers to achieve common goals of our mission statement and purpose through meetings, trainings and other events.
  4. Health Education & Crisis Management: Provide health education to clients regarding HIV disease, especially the importance of medical care and treatment adherence. Provide crisis management to clients as needed.
  5. Administration: Maintain client progress notes in electronic client chart (HERO) and enter data in AIDS Regional Information & Evaluation System (ARIES). Prepares and maintains client clinical charts.
  6. Community Relations: represent the SFAF Black Health CoE at community and SF DPH meetings. Attend meetings of the Consumer Advisory Board (CAB). Participate in semi-monthly case conferences at San Francisco General Hospital.
  7. Other duties as needed.
