Position Purpose

The Pre-Exposure Prophylaxis (PrEP) Navigator provides intensive care coordination, support and services to HIV negative individuals who require assistance in accessing and remaining in PrEP care. This individual will coordinate with the Prevention team to initiate access to PrEP care and the Medical and Pharmacy teams to provide assistance to existing PrEP patients. 

This individual will implement a client-centered approach with individuals throughout the state of Wisconsin, with a targeted focus on those utilizing ARCW PrEP clinics. This position will assist PrEP clients with services such as, but not limited to, health insurance enrollment, PrEP provider identification, appointment scheduling, management of clinical costs, appointment reminders, education on the benefits of PrEP and ongoing maintenance. 

This position will work to empower clients, with a targeted focus on Black Women and Transgender women, to navigate medical and social systems on their own.


Scope of Responsibilities
 
The AIDS Resource Center of Wisconsin (ARCW) is one of the nation’s largest providers of HIV prevention, care and treatment services with a budget approaching $120 million and employing nearly 300 professionals.  Our organization also includes both St. Louis Effort for AIDS (EFA) and Denver’s Rocky Mountain CARES (RMC).

The unique, nationally recognized ARCW HIV Medical Home model of care assures that everyone with HIV has access to medical, dental, mental health and social services and provides the best opportunity for patients to achieve high quality health outcomes.  ARCW also provides aggressive HIV prevention services to gay men, injection drug users and others at the highest risk for HIV infection. 

ARCW operates in the states of WI, CO and MO.  It embraces a Market Culture to increase access to care, enhance quality outcomes and achieve and surpass financial performance expectations and has an aggressive plan to expand into additional states.



Position Responsibilities

  1. Work with Prevention staff and others to identify potential PrEP clients and those in need of PrEP navigation services.
  2. Provide support to HIV negative individuals who are interested in starting PrEP and those already utilizing PrEP care.
    1. Complete screenings to identify readiness for PrEP and other barriers to care. Work with client to identify position solutions.
    2. Assess benefits and insurance, and if needed, provide assistance to access payor programs.
    3. Provide PrEP disease prevention and PrEP adherence education and counseling.
    4. Effectively collaborate with all individuals involved in PrEP care and program delivery for the purposes of supporting high quality PrEP programming.
    5. Make referrals for external services needed to support PrEP care and follow up to ensure service delivery.
    6. Provide ongoing follow up to ensure PrEP adherence.
    7. In some cases, in consultation with the PrEP coordinator, provide transportation to and from PrEP appointments.
  3. Maintain appropriate patient files and timely documentation of services utilizing electronic prevention or medical software.
  4. Serve as a fully functioning member of the Prevention team, able to participate in daily activities and functions to maintain overall ARCW operations.
  5. Provide outreach to relevant communities with a focus on the men who have sex with men of color (MSMOC) community.
  6. Establish working relationships with appropriate community resources in the area, including those providing mental health and social services to the target population.
